C2A’s Community Connections hubs have grown and developed with our participants since 1953. One thing has remained constant: our ongoing passion for understanding the needs of people with disability, their carers and families.

Group and Individual Activities

We have a variety of hub based activities as well as creative and exciting individual external programs. Our hub-based activities include cooking classes with shopping and budgeting, music and dance, art, gaming, health & wellbeing, paddock to plate garden programs and more. Our amazing urban adventure programs take you to adventurous places and points of interest, and fascinating outdoor experiences.

A variety of programs are available during the day, and several activities after hours and on weekends. Including our Mix and Mingles and Funday Fridays Disco and Dinner.

We know that people with disability want the same opportunities as everyone else, and that their ultimate aim is to live a fulfilling life. That is why our hubs have engaging programs that strive to create an inclusive environment. 

 

Our hub based activities include engaging programs in cooking classes, music and dance, art, gaming, exercise, growing vegetables and more. Participants choose programs based on interests and goals. New programs are continually developed. 

 

Our Social and Community Participation programs are also tailored to meet our participants’ goals. They include independent living skills, improving health and wellbeing and building independence. We have created amazing urban adventure programs which take our participants to adventurous places such as the Melbourne Museum, Koala Conservation and Puffing Billy.

Complex Supports

Our Disability Support Workers are highly skilled and trained in working with complex needs.

 

Our complex and high intensity supports we provide are based on individual needs and we will work with you and your loved ones to ensure all supports and activities meet your goals. We follow all Health Care Plans to keep our participants safe.

 

Our experienced Disability Support Workers can assist you with a range of daily living activities like dressing and grooming, personal care, assisting with communication devices, supporting the use of mobility aids, medication management and mealtime management. We also implement Positive Behaviour Support Plans and assist your Behaviour Support Practitioner on how the plan has workedand what could be improved.

Registered NDIS provider

As a registered NDIS provider, Connecting2Australia upholds the specific quality and safeguards requirements of the NDIS Commission. Our quality and safeguarding arrangements align with the NDIS Code of Conduct, including quality improvement and assurance.

Our workers are skilled and trained in high-level, participant-focused outcomes to deliver safe and ethical supports and services to NDIS participants.

Our Integrated Management Systems ensures C2A optimise business disciplines through certification to international Standards in ISO 9001 Quality Management, ISO 14001 Environment, ISO 45001 Occupational Health and Safety and Information Security.
